Mixed-up Index

Unfortunately, the Index for the third edition (first printing) of Television contains numerous errors and omissions. Due to a production error, all references to chapters 12 and 13 are off by one page and, due to a bug in our indexing software, there are several missing entries. We have completely redone the Index for the second printing of the third edition.
